every first Wednesdays of the month at the Asian Bank, 668
N. 44th Street, Suite 123, Phoenix, AZ 85008. Asian Bank is
located just one block South of the 202 Freeway and 44th
Street.

business owners and the soon-to-be entrepreneurs to attend
our monthly mixers. Attendance is free to Chamber members
and first time guests. Repeat guests pay $5/mixer.

Entrepreneur of the Year Award
Criteria for the Nominee
1.
Nominee must be a Filipino-American business owner.
2.
Nominee must submit a proof of business status (i.e. W9 –
Certificate of Tax Status or a Registration of Business at the
Arizona Corporation Commission)
3.
Nominee must meet and understand the Awardees’ Selection
Criteria on page 2
4.
Nominee must submit three Letters of Reference from most
recent or current client stating high level of performance of
service delivery is required.
5.
Nominee must submit the completed Nominee Credential Form on
Page 4. Completing all five items will provide better gains to
become a winner.
6.
Nominee must be nominated by another entity or organization
that has the ability to vouch performance of service delivery.
Nominating party must provide a description of justification
(see page 5) and attest (see page 6) why such nominee will
deserve to become a recipient of the Award.
Nomination must be
received within 60 days from February 10, 2008. Deadline
for Nomination is
April 18, 2008.
All Nomination Forms will be dated when received. Any
Nomination Form received after April 18, 2008 will not be
included.
